NOTICE TO MAGNOLIA MEDICAL CLINIC PATIENTS

Magnolia Medical Clinic is providing notice regarding a mailing error involving correspondence previously sent to certain patients.

Due to an administrative error in preparing a patient mailing, some letters were mailed to the correct patient address but displayed the name of another individual on the envelope only. The correspondence was a general clinic notification and did not contain medical records, diagnoses, treatment information, Social Security numbers, financial information, or other clinical information.

Upon discovering the error, Magnolia Medical Clinic promptly investigated the matter and took corrective measures, including reviewing and correcting the mailing process and reissuing appropriate correspondence to those affected.
Patients who received a letter from Magnolia Medical Clinic displaying a name other than their own, or who have questions regarding this notice, may contact Magnolia Medical Clinic's Privacy Officer at (850) 243-7681 during normal business hours.
