What To expect:

Seeing a doctor for the 1st time or the 100th time can be an intimidating experience. So let us help you lessen the stress by sharing a few tips to help you navigate the process.

We hope that these tips help you get the most out of your first appointment, and we look forward to many more appointments with you in the future!

BEFORE YOUR FIRST VISIT

Have all your medical records from previous doctors sent to our office. We have a Medical Records Release Form for you to fill out that will help with the process.
Prepare a list of questions for your provider.
Prepare a list of all your current medications and dosages, including over counter.
Gather as much of your family medical history as you can remember, including your surgery dates, allergies, and pediatric patients’ immunizations.

DAY OF YOUR VISIT

You will receive a link via text or email with your online registration. Please complete your online patient check in as soon as possible. We ask that you arrive 30 minutes prior to your scheduled appointment time.
Make sure to have your current insurance card and personal identification.
Call ahead of your appointment if you are running late, need to cancel or reschedule.
Bring your medication list as well as your questions.

DURING & AFTER YOUR VISIT

Make sure any questions your provider has, are answered to the best of your ability.
Review your questions with the physician.
Be sure you write down or understand any instructions the physician gives you.
Make sure you receive your prescriptions or know what pharmacy they are sent to
Checkout at the front desk and schedule your next appointment.
